• Eligibility. Answer questions to determine your organization’s eligibility.
• Presenter Organization Information. Complete basic information about your organization.
• Artist Collaborator Information. After accepting the request and logging into SurveyMonkey Apply (see below), the artist will have access to the application and can contribute to this form. Hint: provide the artist access to the application portal as soon as possible.
• Engagement Project Description. You and the artist complete open-ended questions about the project.
• Agreements and Signatures. You and the artist must both review and sign this form. The artist may provide clearance for presenters to sign on their behalf.

1. Within the application, on the left side of the screen, click the “Add Collaborator” button. Enter the email address of the artist collaborator you are adding, set the permissions of the user to “View & Edit,” and provide an optional message.
2. Once the invite is sent, the artist will receive it at the email you provided. Within the email, the artist will be able to join the application or decline the invite.
3. Upon clicking “Join Now” within the email, the artist will be able to either login or complete the registration process, depending on if they already have an account.
4. After accepting the request and logging in, the artist will have access to the application and can begin contributing to the application.
   
   

Important: Please note the following regarding who can see and edit your information. You and the collaborating artist will be able to see all responses within the entire application.

1. Only you will be able to edit information on the Presenter Organization Information form.

2. The Artist Collaborator Information form requests information, including demographics, that should be provided directly by the artist.

3. All parties will have access to edit the Engagement Project Description form and Agreement and Signatures form. You and the artist may wish to agree on who is allowed to edit these forms.
   
   

While completing the application, note that the platform has an auto save feature. You can also select “Save & Continuing Editing” or “Mark as Complete” at the bottom of each form as you proceed through the application. The task bar on the left of your screen will indicate your progress within the application. Instructions to make edits to a completed form can be found here.

The application must be signed by you and the artist. The artist may provide clearance for presenters to sign on their behalf. Instructions on how to sign the application in SurveyMonkey Apply are found here. Upon submission, the presenter and artist will both be emailed to confirm that the application was submitted and signed by both parties.

Preview your application prior to submission by following these instructions.

The “Review” and “Submit” buttons found at the bottom of the task bar will only be unlocked when all tasks are completed. You can preview your application prior to submission. You should be able to edit responses up until the deadline at 5:30 p.m. EST on Monday, November 15, 2021.

Follow these instructions when ready to submit or select the “Submit” button at the bottom of the review screen. Only you, the presenter applicant, has access to submit the application.
